About the role 

Richmond Response and Rehabilitation Team (RRRT) Social Care

RRRT is a well-established and integrated health and social care team, where we work collaboratively to achieve an excellent service for our residents in Richmond. Working across several NHS acute hospital Trusts and community health services, we deliver high quality, effective outcomes though working and supporting each other, within a committed dedicated team.

You will be innovative, engaging and creative in managing your team, helping to deliver safe and well-co-ordinated hospital discharges. You will oversee the effective management of hospital discharges from acute and non-acute hospitals and quickly build positive working relationships with our Health and community partners as a natural communicator.

Our service requires managers to be able to respond to a change in circumstances, due to a hospital admission. Therefore, the ability to think quickly, act calmly and to be decisive is necessary for this role. The successful candidate will deliver effective risk management, with a clear knowledge of legislation and guidance in relation to hospital discharge and Continuing Health Care, to promote well-being and positive outcomes.

This is a hybrid role with a mix of working from the Council office, working from home and visits to hospitals.

Essential Qualifications, Skills and Experience

A professional qualification in Social Work or Occupational Therapy and registration with SWE/HCPC.

Experience and skills in managing people

Leadership and Management training/qualification (or willing to undertake)
